Working on Ben Franklin's adage that an ounce of prevention is worth a pound of cure, Sutter Health pioneered an innovative program to help patients manage with ongoing health conditions. The Sutter Continuum Case Management department provides coordination of services to help patients maintain the best health possible. In 2003 the department was awarded the inaugural Franklin Award of Distinction by the Joint Commission of Accreditation of Healthcare Organizations (JCAHO) and the American Case Management Association for improving quality across the continuum of care.

Sutter's Continuum Case Management Department also offers special resources for Sutter hospital patients and those who need extended care after they leave the hospital. Sutter Case Managers work with physicians, patients and families to determine the best next steps for each patient, which may include referrals to skilled nursing or custodial care facilities or to community programs. Patients may also be enrolled in one of the ongoing management programs for care coordination, heart failure, anticoagulation medications or asthma.

Sutter Acute Care Case Management and Social Work Programs

Patients who need assistance in transition from hospital care, to a skilled nursing or custodial care facility, or back to their homes receive visits from Sutter's nurse case manager or social worker who helps support patients and families through the decision-making process and as they adjust to ongoing care of their conditions...(more)
