People with one or more chronic health conditions like diabetes, arthritis, etc. often face challenges that can be confusing and sometimes overwhelming. The Sutter Care Coordination Program (SCCP) is designed to make it as easy as possible for patients to take good care of their health and to gain access to Sutter services and community resources they need to help them live full, productive lives. Every aspect of the person’s well-being receives attention, including physical, mental, emotional, educational and social support.

Designed for chronically ill patients, frail seniors and patients who have been recently hospitalized, SCCP teams work with the Sutter patients, families and primary care providers to extend support as needed. SCCP team members include registered nurse case managers, medical social workers, and healthcare coordinators who combine their knowledge and skill for the patient's benefit.

The team helps patients understand their health conditions and have access to the services they need. The program also provides periodic monitoring to ensure patients are following their doctors' instructions on taking medication as directed.

Each patient begins by receiving a comprehensive telephone, office or in-home assessment by a nurse case manager or medical social worker. Once the team determines the patient's specific needs, they create a detailed treatment plan. The healthcare coordinator teaches the patient about the disease process and helps match up the patient with Sutter services. The coordinator may also connect patients with community resources for meals, personal care and other assistance.

The Sutter Care Coordination Program was the first of its kind to receive Joint Commission on Accreditation of Healthcare Organizations (JCAHO) Disease-Specific Certification. To participate in this program patients must have a Sutter Medical Group, Sutter West Medical Group or Sutter Independent Physician. There is no fee to the patient.
