The Sutter Diabetes Management Program assists patients with diabetes by providing ongoing monitoring and tools necessary to take control of their condition. Specially trained staff provides consultation, support and information through telephone contact. By working with your physician the team will help coordinate services that are very important to preventing complications associated with diabetes. The team also mails reminders when your physician wants you to have important procedures, such as blood work or dilated eye examination.

Your physician, nurses, pharmacist, registered dietician, certified diabetic educators, care management representative, and you make up your diabetes care team. While all are important, you make the biggest difference. Our team will help you to understand your condition, the medications you take and the things that you can do to make your condition better. We will work with you and your physician to help develop a personalized diabetes care plan. Our goal is to provide the information and support needed to help you keep yourself healthy. We maintain regular contact with your doctor regarding progress and changes in your condition.

To participate in this program, patients must have a Sutter Medical Group, Sutter West Medical Group or Sutter Independent Physician. There is no fee to the patient. For information on the Sutter Diabetes Management Program, please call 1-800-544-1808 option 5.
