The program is designed to help patients with heart failure maintain the highest level of health. Sutter program staff provides a link between the patient and their doctor or cardiologist by maintaining frequent scheduled phone contact and monitoring signs and symptoms. Staying on top of what's happening enables problems to be detected at their onset, medication to be adjusted, and program staff to coordinate any follow-up care needed.

Regular contact also helps ensure patients understand and follow their doctor's directions for diet, exercise and medication. Staff also helps educate each patient about heart failure symptoms and progress so that patients have a clear understanding of why each step of their treatment plan is important to maintaining overall health. For patients who are not able to get out often, regular contact also provides important emotional support and encouragement that helps them stay motivated in caring for themselves.

At the start of the program, patients receive education and tools to help them better manage their health. Telemanagement staff remains available to each patient should questions or problems arise. Family members and care givers are also encouraged to take part in the education so that they are able to support the patient's self-care routines.

The Sutter Heart Failure Telemanagement Program has maintained Joint Commission on Accreditation of Healthcare Organizations (JCAHO) Disease-Specific Certification since 2003. To participate in this program patients must have a Sutter Medical Group, Sutter West Medical Group or Sutter Independent Physician. There is no fee to the patient.
