The Sutter Senior Assessment clinic provides a chance to meet with a geriatrician, a physician who specializes in caring for seniors, to address chronic illnesses, concerns or problems affecting your ability to manage well or live independently. The clinic will help you and your doctors develop and plan for your over all health and well being. All the information from the clinic will be shared with your primary care physician.
Common problems may include: - Falls or fear of falling
- Difficulty managinging multiple medications
- Incontinence or loss of urine or bowel function
- Problems with memory
- Difficulty with taking care of daily needs, such as walking, dressing or bathing
- Unplanned weight loss
- Needing more help at home or needing help with finding a place that will provide needed care services
What to expect at the clinic: - Review medical history and current problems.
- Review of all vitamins, over-the-counter and prescription medications
(bring them all to your appointment) - Physical exam
- Discussion of daily activities and symptoms that may be common in people as they get older.
A physician's referral is required for insurance company coverage or you may self refer and pay for the appointment.
Family members or care givers are welcome.
Welcome to Medicare Visit The clinic also provides new Medicare HMO and traditional Medicare patients the Welcome to Medicare Visit, a new benefit available for patients to use within the first 6 months of their Medicare eligibility. The visit will include a comprehensive history, medication review, exam, assessment with indicated screening tools and a patient-specific review of health care maintenance.
